About the Role

The postholder works closely with the Health and Wellbeing team and acts as the lead deliverer for new and existing strands of the Bridging the Blues Programmes.
Key Responsibilities

- Prepare delivery programmes and session plans that support project outcomes, ensuring participation forms, recording sheets and registers are completed accurately and entered into the Foundation's monitoring and evaluation systems.
- Ensure all health and safety and safeguarding procedures are implemented prior to any activity starting, quality assuring casual staff delivery and confirming that all participants have undertaken the appropriate Physical Activity Readiness Questionnaire and exercise referral screening calls.
- Support monitoring and evaluation for all Population Health projects in accordance with each funder's needs, ensuring all data is captured at session and input onto Salesforce.
- Take responsibility for the safeguarding of participants across all Population Health programmes, liaising and reporting into the Safeguarding Lead for Health and Wellbeing and associated partner Safeguarding Teams to ensure a clear line of reporting to the CFC Safeguarding Hub.
- Achieve all KPIs and outcomes as agreed with the funding provider and the Population Health Senior Officer, and complete and submit project reports at six-month intervals or as otherwise required internally or by funders.
- Capture success stories through working with Chelsea FC Foundation and Chelsea FC media staff.
- Act as a positive role model, creating a safe, positive and fun environment in which to motivate and encourage the community to live a healthy and active lifestyle.
- Embody the club's BLUE behaviours (Brave, Lead, Unity, Edge), adhere to club policies and procedures including Health & Safety, Financial Authorisation, Confidentiality and GDPR, and act as an ambassador for diversity, equality and inclusion.
